2012 Spring Programs begin 3/3/12
We require three forms to complete your enrollment.
Participants must complete all of the following forms:
(Excluding Saturday Night Out)
Profile - required once per calendar year
Health History - required once per calendar year
Program Registration Forms:
Current Rider Re-Registration
Year Round Registration (including first time riders)
Optional:
Scholarship Assistance Form
Financial aid may be available based on demonstrated need
